How to Hit Financial Goals Without Becoming a “Dental Factory”

A dentist wearing glasses and a white uniform stands with her arms crossed in a dental office.

It is one of the most frustrating puzzles in private practice: the front door is swinging, new patients are flowing in, and the schedule looks full. Yet, when you look at the end-of-month reports, your associate production is stagnant.

Where is the disconnect?

In a recent Toolbox Tuesday podcast episode, Jennifer Paxton tackled this exact issue. Too often, owner-doctors want more production, while associate doctors feel overworked, burnt out, and worried that pushing the numbers means compromising clinical care.

But here is the good news: hitting high financial goals and delivering exceptional patient care are not mutually exclusive. You do not have to turn your private practice into a high-volume clinic to see your numbers thrive. It all comes down to alignment, clarity, and breaking big numbers down into bite-sized daily actions.

1. Focus on “The Basics”

When we talk about increasing associate production, people often assume we mean pushing cosmetic makeovers or over-diagnosing treatment. That could not be further from the truth. In a healthy private practice, sustainable production is found by consistently diagnosing and treating the basics:

  • Cavities
  • Cracks
  • Infections

There is already a plethora of this essential care sitting right in your charts. Production increases when the team becomes highly calibrated at identifying, presenting, and scheduling the treatment that patients actually need to stay healthy.

2. Shift from the What to the Why

“Patients can’t afford it.” It’s the oldest objection in dental history, surviving recessions, pandemics, and inflation. But let’s be real: very few people wake up excited to spend $1,500 on a crown.

To overcome the financial barrier, the clinical team must stop just telling the patient what they need and start clearly educating them on why they need it.

Instead of saying, “You need a crown on tooth number 3,” explain, “There is a fracture line running down this tooth. If we don’t protect it with a crown now, the tooth is at risk of breaking entirely, which could mean a root canal or an extraction down the road.”

When patients understand the visual or functional why, the value outpaces the cost.

3. Reverse Engineer the Daily Schedule

An annual production goal can feel incredibly overwhelming to an associate doctor. If you tell an associate they need to produce $800,000 this year, they might envision a nightmare schedule of 30 patients spread across five columns, leaving them physically decimated.

Instead, reverse engineer the goal into a daily procedure mix.

Let’s say an associate’s daily target breaks down to $4,000. When you visually map that out on a schedule template, it suddenly looks completely manageable:

The Daily Target Procedure Mix

  • 2 Crowns
  • 3 Fillings
  • 2 New Patient Exams

When broken down this way, the associate can breathe a sigh of relief. It’s not about rushing through a sea of patients; it’s about maximizing the few production slots you have.

4. Total Transparency and the “Red/Green” Scorecard

You cannot manage what you do not track. Successful practices eliminate the mystery around numbers by implementing custom doctor scorecards that are updated weekly and shared transparently.

An effective scorecard tracks metrics beyond just raw production, focusing heavily on true case acceptance.

  • The Trap: A doctor presents a $10,000 treatment plan. The patient schedules one $1,200 crown. Many offices mistakenly celebrate this as 100% case acceptance because “the patient scheduled something.”
  • The Reality: There is still $8,800 of outstanding, necessary dentistry left on the table. Track case acceptance by the dollar amount scheduled so you know exactly where the ongoing opportunity lies.

By using simple conditional formatting—where hitting a weekly goal turns the cell green and falling short turns it red—the entire team gets immediate, objective feedback. If a doctor has an incredible week, they can instantly share what worked so the rest of the team can duplicate that success.
